-- Valid Pass, i.e. Employment Pass, S Pass, Work Permit, Dependent's Pass or Long Term Visit Pass (Valid for at least 9 months)

Proof of Income

of Principal Applicant

For salaried employees
-- Latest 3 months' computerised/electronic payslip; or
-- Latest 12 months' CPF contribution statement; or
-- Latest IRAS Notice of Assessment and latest month electronic payslip

For commission-based employees/self-employed
-- Latest 12 months' CPF contribution statement; or
-- Latest 2 years' Income Tax Notice of Assessment

Proof of Address

For Singapore Citizens and Permanent Residents
-- If your billing address differs from the address on your NRIC, please provide a Proof of Address i.e. Latest bank statement, telephone or utility bill.
-- The address on Proof of Address must match the billing address in the application

For foreigners
-- Please submit a Proof of Address i.e. Latest bank statement, telephone or utility bill.
